What this license covers

This license governs all paid SignTheChart template products and downloadable materials, including template kits, message and documentation templates, dot phrases, building blocks, worksheets, calculators, field guides, ebooks, and web app tools (the "Materials").

This license is part of, and governed by, our Terms of Purchase. If anything on this page appears to conflict with the Terms of Purchase, the Terms of Purchase control. Our Disclaimer also applies to your use of the Materials.

Ownership

The Materials were created by Candice Elam, DNP, FNP-C and are owned by Nursing Current LLC d/b/a SignTheChart ("SignTheChart"). They are protected by copyright, trademark, trade secret, and other intellectual property law.

Purchasing a product does not transfer ownership of anything to you. It grants you the license described below and nothing more.

Your license

Subject to your payment in full and your compliance with this license and our Terms of Purchase, SignTheChart grants you a single-user, personal, non-exclusive, non-transferable, non-sublicensable, revocable license to use the Materials in your own clinical practice.

You may:

Adapt and customize the Materials for your own patient communication and documentation.
Use the Materials in the care of patients you personally treat.
Load the Materials into your own individual, personal template library, smart phrase set, or user account within your employer's electronic health record, to the extent your employer permits it.
Print or save copies for your own reference.

You may not:

Resell, share, send, forward, gift, sublicense, assign, transfer, redistribute, or post the Materials publicly, in whole or in part, in original or modified form.
Load, publish, or otherwise make the Materials available in any shared, group, departmental, enterprise, institutional, or organizational template library, intranet, drive, folder, or system where clinicians other than you can access them.
Share your account credentials or allow anyone else to access the Materials using them.
Teach, present, train, or deliver the Materials or the frameworks behind them to others, whether or not for a fee.
Use the Materials to create a competing product, kit, course, worksheet, guide, tool, or training for sale or distribution.
Input the Materials into any artificial intelligence or automated tool for the purpose of generating derivative or competing materials, or to train, fine-tune, or ground any AI model.
Remove, alter, or obscure any copyright notice, trademark, attribution, or licensing language contained in the Materials.

Any use beyond your own personal clinical use requires the prior written consent of SignTheChart.

Your employer's systems and policies

You are solely responsible for determining whether your employer, health system, or institution permits you to load or use outside material in its electronic health record or other systems, and for complying with its information technology, intellectual property, security, and template governance policies. Some employers restrict or prohibit this. Confirm before you build.

Any documentation you create for your own patients using the Materials remains subject to your employer's ownership and record-keeping arrangements. SignTheChart claims no interest in your patient records.

Your professional judgment

The Materials are documentation drafting aids and structural starting points. They are not medical advice, clinical decision support, clinical protocols, or a standard of care, and they do not establish medical necessity or support any billing or coding level.

You retain full and independent professional judgment. You are responsible for reviewing, editing, and verifying every template before use, for confirming it is accurate and current for the specific patient, and for ensuring your documentation reflects the care you actually provided. Institutional, group, and academic licensing

If you want to use the Materials across a practice, department, residency program, health system, or academic institution, or to provide them to more than one clinician, you need a separate written license from us.

We offer institutional and academic licensing. Contact support@SignTheChart.com and tell us what you have in mind.

Termination

This license terminates automatically and immediately, without refund, if you breach it, breach our Terms of Purchase, or initiate a chargeback or payment dispute.

On termination you must stop all use of the Materials and destroy every copy in your possession or control, including any copy you have loaded into an electronic health record or other system.

Our ownership, your obligations regarding confidentiality and non-redistribution, and any accrued liability survive termination.

Reporting misuse and requesting permission

To request permission for any use beyond your own personal clinical use, or to report unauthorized sharing or redistribution of the Materials, contact support@SignTheChart.com.

Governing law

This license is governed by the laws of the State of New Jersey, and any dispute is subject to the dispute resolution, jurisdiction, and venue provisions of our Terms of Purchase.
