3. Clinical documentation materials and your professional judgment

Some SignTheChart products are clinical documentation materials. This includes note structures, dot phrases and text expansion content, history and physical frameworks, assessment and plan language, order set companions, referral and prior authorization letter templates, and patient message templates.

These materials are drafting aids and structural starting points. They are not clinical decision support tools, not clinical protocols, and not a substitute for your own evaluation of the patient in front of you.

By purchasing or using any SignTheChart clinical documentation material, you acknowledge and agree that:

  • You are a licensed clinician acting within your own scope of practice, licensure, and applicable state and federal law, and you are solely responsible for confirming that you are.

  • You retain full and independent professional judgment and full responsibility for every clinical decision you make and every entry you place in a patient record.

  • You are responsible for reviewing, editing, and verifying every template before use, and for confirming that its content is accurate for the specific patient, current, and consistent with prevailing clinical guidelines and your institution's standards.

  • Clinical guidelines, drug information, coverage criteria, and regulatory requirements change. SignTheChart does not warrant that any material reflects the most current guidance at the time you use it, and has no obligation to update, correct, or notify you of changes to any material after purchase.

  • Documentation must reflect the care you actually provided. You will not use any SignTheChart material to document services, findings, history, examination elements, or medical decision making that did not occur.

4. Billing, coding, and compliance

SignTheChart products are not billing, coding, reimbursement, or compliance advice, and are not designed or represented as tools to increase billing levels, reimbursement, or productivity credit.

No SignTheChart material establishes medical necessity, supports any particular evaluation and management level, or guarantees payment, coverage, prior authorization approval, or a favorable audit result. You are solely responsible for the accuracy of your documentation and coding and for compliance with all applicable payer requirements, federal and state law, and your employer's compliance policies.

5. Employer systems, electronic health records, and institutional policy

SignTheChart products are licensed to you as an individual clinician for your own professional use.

You are solely responsible for determining whether your employer, health system, or institution permits you to load, build, import, or otherwise use SignTheChart material within its electronic health record or other systems, and for complying with its information technology, intellectual property, security, and template governance policies. Some employers restrict or prohibit outside content in shared systems.

Unless you have our separate written license, you may not load SignTheChart material into a shared, departmental, enterprise, or institutional template library, or otherwise make it available to other clinicians. SignTheChart is not affiliated with, endorsed by, or sponsored by Epic Systems Corporation or any other electronic health record vendor. Any product names or trademarks referenced are the property of their respective owners and are used only to describe compatibility or function.

6. No legal advice
